Required Paperwork and Forms

All necessary permission paperwork and forms must be completed and submitted by the first day of Summer Marching Band Camp. For percussion and color guard members, forms are due July 15. For all other band members, the forms are due July 29. Ensuring these documents are turned in on time is crucial for participation in band camp and the upcoming season. Students will not be able to participate in outdoor rehearsal without a physical on file.

2024-25 Physical Evaluation:
Please fill out the first page of the form. At a doctor's evaluation, the doctor will fill out and complete the second page. Evaluations can be done at your primary care doctor, walk-in clinic (such as CVS or Walgreens), or an urgent care that performs physicals.
Fine Arts Field Trip:
Complete form and sign.
Parent  Permission for School-Sponsored Activity:
Please fill out this entire form. Campus will be Cy Creek HS or CCHS. You will check off both With AND Without District Transportation. On the line for activity, please list: Football games, Marching Band Rehearsals,  Performances, UIL and Non-UIL Events.
Parent/Student UIL Marching Band Acknowledgement Form:
Both parent and student will need to read and sign.
Electrocardiogram Screen Consent and Release of Liability:
Complete form and sign.
